At Find Wellbeing, we provide virtual care support those who care, those who cope, and those who matter most.

We’re here for:

What to expect when you contact us

Find wellbeing wants only the best for your loved ones.
Here is a step-by-step process when reaching out to us.

Non-NDIS participant

Once you have selected the type of care or service you require, then fill in the form and make an appointment. One of our team will give you a call to discuss your needs.

Note: If you need urgent help or support, please call your support coordinate, GP, 000 or Lifeline.

3 If you booked a “Virtual Care 4 – Non-NDIS call back, one of our team will call you to contact a brief intake meeting and discuss your current circumstances. This will help us identify what services would be most appropriate for you and your loved ones
4 We will aim to tailor a program that best suits you and introduce you to one of virtual care support staff who will
partner with you on your wellbeing journey.

For NDIS Participants

3 If you booked a “Virtual Care 4 – NDIS call back, one of our team will call you to contact a brief intake meeting and discuss your current circumstances. This will help us identify what services would be most appropriate for you and your loved ones.
4 During the initial intake meeting we will discuss your plan, ask for information regarding your plan manager & support coordinator and whether or not you wish to use our plan manager services, and discuss our fees and how we will charge you as a NDIS participant. We will also set up a secure portal where you can upload your provider invoices. We can also provide your providers with a secure portal to upload their invoices directly to us.

Emotional Support

is significant for the mental health of our older loved ones, and it is also a medical necessity. Thankfully, there are many concrete ways to provide the necessary support to help the ones you love thrive.

 Depression and anxiety disorders. A recent report shows that almost 60% of those living in residential aged care have a mental health diagnosis. It also addresses the need to involve families in meaningful ways to assist their relatives living in residential care.

Forming strong and meaningful relationships

We help build deep and meaningful connections with clients by fostering strong relationships. Engaging in regular conversations with them, taking the time to actively listen to their thoughts, feelings, and stories. Show genuine interest in their lives by asking open-ended questions that encourage them to share more about themselves. Remember important details they’ve shared in previous conversations and follow up on those topics in future interactions. This demonstrates that you value their presence and are invested in getting to know them on a personal level.

Confidentiality

Your privacy is extremely important to us. Find Wellbeing is committed to preserving and upholding your rights to privacy and confidentiality. We are bound by the Privacy Act 1998 and the Australian Privacy Principles.

We won’t share your information with other agencies or external parties without your consent.  Your information is only shared under exceptional circumstances such as where your safety or the safety of others is at serious risk, or in accordance with the law.

Note:

  • We will never be the primary support person. Our role is to support the primary care giver/provider.
  • We are not mental health providers e.g., counsellors or psychologists. We will provide weekly reports to the primary care giver/provider. It is up to the primary care giver/provider to determine if the client requires additional support.
  • We are not medically trained. We do not provide any medical diagnosis or prognosis.
