STEP 1

Fill, sign and submit this digital service agreement from to Find Wellbeing’s national team for processing.

Receive your welcome pack via email.

STEP 2

Call the NDIS National Contact Centre on 1800 800 110 and nominate Find Wellbeing as your “plan manager” and “my provider”.

Receive your secure portal login and begin the process process of submitting provider invoices for payment. If you have questions, feel free to call 1300 88 38 30 to talk with a member of our team.

Please call the NDIS National Contact Center on 1800 800 110 (available 8am-8pm local time) and nominate FindWellBeing as your plan manager and my provider.

When making the request, please ensure the NDIA Service Officer uses the correct Provider Registration information as outlined below:
