Your Rights and Responsibilities
Find Wellbeing is a virtual care support service. Find Wellbeing provides support to our ageing loved ones, those with mobility issues, and anyone needing additional support.
This document explains your rights and responsibilities as a client of Find Wellbeing. It is important that you know and understand these rights so you are fully informed before you start using the service. It is also important for you to know how to give feedback about our service.
Your Rights as Find Wellbeing Client
As a client of Find Wellbeing you should expect to:
be treated with respect, dignity, and courtesy regardless of age, disability, cultural and linguistic background, gender, sexual orientation, socio-economic status, and religious or spiritual beliefs.
- have fair and equal access to Find Wellbeing services.
- receive services that comply with appropriate standards of professionalism, competency and accountability.
- be able to refuse any, or all, assessment and care that is offered.
- be provided with information about your wellbeing and plans and be involved in decision-making regarding these options.
- have your privacy and confidentiality protected, within the limits imposed by law.
- access your records by request, in accordance with the Privacy Act 1988 and the Freedom of Information Act of 1982. A copy of our Privacy Policy is available at https://www.findwellbeing.coma.u/privacy or upon request.
Your Responsibilities as a Find Wellbeing Client
In return, Find Wellbeing asks that you: - respect the privacy and confidentiality of other clients and staff
- treat other Find Wellbeing clients and staff in a respectful manner
- ensure that you are not under the influence of alcohol or other drugs, and/or not
behaving in a way which makes delivery of service difficult or dangerous. - do not intentionally spread misinformation, or provide incorrect or misleading advice, that prevents or deters others from accessing Find Wellbeing services.
- honor agreements made with Find Wellbeing about service and provision of care.
Feedback and Complaints
Find Wellbeing welcomes and values your feedback as it assists to improve the service we offer you. Feedback or health complaints can be provided in the following ways: - Contact Find Wellbeing by calling 1300883830 open from 8am-5pm, Mondays- Friday
- Visit https://www.findwellbeing.gov.au/about/contact-us and leave us a message through our webform
- Contact your GP, health officer, NDIS case manager or support worker, or Aged Care manager
